{"id":1404,"date":"2018-03-08T13:20:17","date_gmt":"2018-03-08T18:20:17","guid":{"rendered":"http:\/\/blog.uvm.edu\/si-mba\/?p=1404"},"modified":"2018-03-08T13:22:58","modified_gmt":"2018-03-08T18:22:58","slug":"the-business-of-health-care-delivery-the-social-determinants-of-health","status":"publish","type":"post","link":"https:\/\/blog.uvm.edu\/si-mba\/2018\/03\/08\/the-business-of-health-care-delivery-the-social-determinants-of-health\/","title":{"rendered":"The Business of Health Care Delivery: The Social Determinants of Health"},"content":{"rendered":"<p><em>This post was written by Gregory Paylor &#8217;18<\/em><\/p>\n<p><span style=\"font-weight: 400\">In the US, health insurance coverage was broadened and expanded under <\/span><a href=\"https:\/\/www.healthcare.gov\/glossary\/affordable-care-act\/\"><span style=\"font-weight: 400\">The Affordable Care Act<\/span><\/a><span style=\"font-weight: 400\">. \u00a0While this reduced the total uninsured population, cost per unit of care went unaddressed and the model of healthcare delivery has remained largely unchanged. \u00a0Only recently have we begun to see <\/span><a href=\"https:\/\/www.cms.gov\/Medicare\/Quality-Initiatives-Patient-Assessment-Instruments\/Value-Based-Programs\/Value-Based-Programs.html\"><span style=\"font-weight: 400\">payment model initiatives<\/span><\/a><span style=\"font-weight: 400\"> attempting to address healthcare payment reform and improvement to patient outcomes. \u00a0Because of this, insurers have been looking for other ways to reduce downstream healthcare spending. \u00a0This is where the Social Determinants of Health (SDOH) come into play.<\/span><\/p>\n<p><span style=\"font-weight: 400\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-1405 alignleft\" src=\"http:\/\/blog.uvm.edu\/si-mba\/files\/2018\/03\/SDOH_m.png\" alt=\"\" width=\"236\" height=\"225\" \/>The <\/span><a href=\"http:\/\/www.who.int\/social_determinants\/sdh_definition\/en\/\"><span style=\"font-weight: 400\">World Health Organization (WHO)<\/span><\/a><span style=\"font-weight: 400\"> defines SDOH as &#8220;<\/span><i><span style=\"font-weight: 400\">the conditions in which people are born, grow, live, work and age. These circumstances are shaped by the distribution of money, power and resources at global, national and local levels. The social determinants of health are mostly responsible for health inequities &#8211; the unfair and avoidable differences in health status seen within and between countries.&#8221; \u00a0<\/span><\/i><span style=\"font-weight: 400\">Examples of SDOH include: safe housing, food availability, segregation, exposure to crime, presence of trash, transportation options, and the natural environment.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Massachusetts, New York, Oregon, Utah, and Vermont are all \u201c<\/span><a href=\"https:\/\/www.healthaffairs.org\/do\/10.1377\/hblog20170907.061853\/full\/\"><i><span style=\"font-weight: 400\">testing strategies<\/span><\/i><\/a><i><span style=\"font-weight: 400\"> not only to link Medicaid and social services, but also to use Medicaid funds to actually deliver supportive services that affect social determinants of health.
